Telehealth uses technology to monitor your health remotely, so you can stay safely at home while receiving the support you need.
It can help you avoid unnecessary hospital visits, stay independent, and receive regular clinical monitoring.
NHS @Home service
Provided in partnership with One Health Lewisham and Doccla.
NHS @Home is a free service that allows patients in Lewisham to receive care at home instead of in hospital. It’s safe, easy to use, and supports recovery from illness with daily clinical checks.
How it works
- you’ll receive a home monitoring kit with a mobile phone, pulse oximeter, thermometer, and blood pressure cuff
- you’ll be asked to check your readings up to three times a day and send them using the phone provided
- a team of clinicians monitors your results and may contact you if action is needed
- you can stay on the service for up to 14 days or longer if necessary.
This service supports people with conditions that need short-term monitoring but not hospital admission. It’s especially useful after discharge from hospital or if your GP is concerned about your condition.
Additional support may include
- face-to-face visits if needed
- mobile ECG or X-ray
- prescriptions delivered to your home

Video appointments
