Helping you stay well, safe and informed Whether you’re looking after your health, recovering from illness, or supporting someone else - we’re here to help.

What we can help with

You can find support for:

  • mental health and emotional wellbeing
  • help after leaving hospital
  • advice about food, smoking, and staying healthy
  • help to manage long-term health conditions
  • finding local health services
  • support for carers
  • keeping safe from abuse or neglect

We also offer advice if you’re not sure what support you need.

Mental health support

If you’re struggling with your mental health, there are local services that can help with:

  • anxiety or depression
  • feeling stressed or overwhelmed
  • support in a crisis
  • talking therapies
  • support groups and drop-ins
  • get help with your mental health

Help when leaving hospital

If you or someone you care for is coming home from hospital, we can help with:

  • care at home
  • equipment or adaptations
  • reablement or short-term support
  • transport and follow-up care
  • support when leaving hospital

Staying healthy

The Council has services to help you:

  • stop smoking
  • eat well
  • stay active
  • manage your weight
  • look after your heart

Long-term health conditions

If you live with a condition like diabetes, arthritis, or heart disease, we can help you manage it.

We’ll support you to:

  • understand your condition
  • access the right medication and services
  • live as independently as possible
  • find local support groups

Support for carers

If you look after someone else, you may be entitled to:

  • a carer’s assessment
  • help with your own wellbeing
  • breaks from caring
  • financial advice or support
