How to apply

Log in

To apply for a licence, you need to log into your My Lewisham account. If you don’t have one already, click 'Apply for a chaperone licence’, then choose 'register' and follow the instructions to create your account.

What information you need

To complete the form, you need to have the following information to hand:

• your past employment details

• details of two people who can give you a reference

• an electronic passport-size photo of yourself.

You also need to upload a scanned copy of a signed declaration for this application.

After you apply

If we don't receive your references and DBS documents within 28 days, we will close your application.
