How to register for the Be Active scheme
Where to register
You can register at any of the Leisure centres that offer the Be Active scheme.
Evidence needed
To register for the Be Active scheme, you will need:

proof of your name, address and date of birth

any extra documents listed below
Lewisham residents aged 16+ living in households receiving benefits
One of these:
-
a letter from us showing that you receive council tax reduction, housing benefit or universal credit
-
a letter from HMRC showing that you receive tax credits (not child tax credit)
-
a letter from DWP showing that you receive one of:
- income support
- jobseeker's allowance
- employment and support allowance universal credit
Students aged 16+ in full-time education and living or studying in Lewisham
A letter from your sixth form, college or university confirming that either:

you are a full-time student in Lewisham or
-
you live in Lewisham but are a full-time student outside of the borough
Lewisham residents aged 16+ with a disability
Proof of your name and address, plus one of these:
-
your Blue Badge
-
your disability Freedom Pass
-
a letter proving that you get one of:
- disability living allowance
- attendance allowance
- personal independence payments
Carers (professional and unpaid) of disabled adults who are Lewisham residents
Proof of your name and address, plus one of these:

a letter from the person you care for, nominating you
-
a letter from the care agency you work for, nominating you.
The person you care for must be:
-
registered for Be Active because they have a disability
-
unable to use services without your support
Carers of disabled children who are Lewisham residents
Proof of the child's disability.
The child:

must have a Lewisham library card
-
does not need to be registered for Be Active
