What we do

The Challenging Needs Team supports adults who have a learning disability and behaviours that challenge services. The service applies a person-centred, flexible, holistic approach to empower the service uses to meet their full potential in their everyday lives. The goal is to achieve enhanced community presence, choice, personal competence, respect and community participation rather than behavioural change in isolation.

What information we collect from you

We may collect:

• name
• address
• date of birth
• ethnicity
• relationships information (carer, next of kin, etc)
• health information
• medication information
• referral information (including behavioural information)
• other agencies involved
• case history.

• health agencies
• adult social care
• London Ambulance service
• London Fire Brigade
• police.
