Privacy notice - Lewisham Youth Service
Lewisham Youth Service offers an array of youth work activities to children and young people aged 8-19 (up to 25 with Special Education Needs and Disabilities). Activities provided include sports, arts, crafts, culture, cooking, employment support, play (free and imaginative), street-based support including mentoring. Services offered are open access and targeted. Targeted services are themed-based group work and one to one support. Open access services are spread across youth centres and adventure playgrounds dotted around the borough, delivered during two to three afternoon and evening sessions a week per centre.
The information we collect from you may include:
- name
- address
- email address
- telephone number, of both the young person and a parent/guardian
- school/education provision (if under 16 years old), and if eligible for free school meals
- date of birth
- ethnicity
- gender
- registration of attendance at youth club sessions
- information about medical or health conditions, including whether or not young person has a disability and/or allergy for which we need to make reasonable
- adjustments
- relevant assessments such as early help assessments, team around the family minutes, educational health care plans (where relevant)
- additional needs and diagnoses (e.g. regarding mental health, special educational needs, communication and social needs, disabilities)
- services working with the family (both now and historically to avoid duplicating support and repeating intervention).
- financial information (if relevant to the family’s request for support)
- details of doctor’s surgery
- education/employment status (for 16 years old and over)
- interests and hobbies
- goals for achievement through the service provision
- additional needs
We use a membership form to collect this data, which is then uploaded into Views, a fully GDPR compliant data management system, run by data processing company Substance. Their own data protection policies can be found on the Substance website.
Agencies we might share the information with
We work with a range of difference services and organisations who may already know the child, young person and family. Where it will be useful to talk with other services and share information, we seek consent from the parent/carer.
If there are safeguarding concerns for a child of family, we will share information with relevant agencies including the Police and Children's Social Care
Withdrawing consent
The information you provide to Lewisham Youth Service will only be shared with your permission. If a child or young person receives targeted level of support from Lewisham Youth Service, we may share their information with relevant services to make sure the young person and/or family/carer/guardian get the appropriate level of support. You can withdraw consent at any point during intervention and speak with your Youth Worker about how your information is used.
