Reviews

What happens in your review.

Reviewing your care and support plan

  • first Review – If you start a new service or make changes to your plan, it will be reviewed within 6–8 weeks
  • annual Reviews – If your plan is stable, it will be reviewed at least once a year (every 12 months)
  • extra Reviews – If your needs change before your next review, a risk assessment will be done to see if more support is needed.

These reviews follow Section 27 of the Care Act 2014, which ensures that everyone with a Care and Support Plan has the chance to reflect on:

  • what is working well
  • what is not working
  • what needs to change

If you feel you need a review sooner, you can ask for one at any time.

What will be checked in a review?

During a review of your care and support plan, a professional will ask questions to see if your plan is still right for you.

  • have your needs or circumstances changed?
  • is your plan working well, or does anything need to change?
  • have you achieved the goals in your plan, or do you need more support?
  • do you have any new goals or things you want to do?
  • could anything be improved to help you reach your goals?
  • is your personal budget enough to meet your needs?
  • is the way your budget is managed still the best option for you? (e.g., should direct payments be considered?)
  • has anything changed in your family, friends, or community support that might affect your plan?
  • are there any new risks to your safety, such as abuse or neglect?
  • are you, your carer, or your advocate happy with your plan?

Who can be involved?

You, your carer, your family, or an advocate (someone who helps you express your views).
