Special needs nursing for children and young people with SEND

Find out about specialist care services for children and young people with special educational needs and disabilities (SEND).

The special needs nursing team offers specialist health visiting support and special needs nursing to children and young people aged up to 19 years old who have complex learning and physical needs. 

The continuing care service is for children who need a specific care package and have complex needs due to:

  • disability
  • accident
  • illness, including life threatening or life-limiting conditions

The Continuing Care service takes a holistic approach to address the health and disability needs of your child in their home.

The service will assess your child and a panel will decide if they are eligible. If your child is eligible, they will get an individual care package that meets their needs.

As your child develops, Continuing Care will review and assess your child to make sure that they continue to get the right care. Both you and your child will be involved in the development of the care package.
