Collaborative care packages: We have 13 years of experience of working with LA support planners, re-enablement teams and social workers to ensure a seamless transition between services. Community healthcare: We are proactive in preventing deterioration of physical and mental health by taking the lead in identifying healthcare needs (we believe this is the responsibility of every professional who comes into contact with the SU) and make direct contact to secure appropriate support. Examples include: • Chiropody • Dietician • Falls prevention service. Primary Care: We foster a positive relationship with each SU’s primary care team, such as their GP and dentist.
