An introduction to the services available.
Support for young people transitioning from children’s services to ensure they receive the help they need to live independently, access education or employment, and maintain their wellbeing.
Who we support
We assist young people who:
- have a learning disability, physical disability, autism, or other additional needs
- have received support from Children’s Social Care and need ongoing care as an adult
- may require an assessment under the Care Act 2014 to determine eligibility for Adult Social Care services
How we can help
We work with young people and their families to provide:
- advice and information – helping you understand the services available to you
- assessment and planning – identifying the right level of support for your needs
- support for independent living – guidance on housing, daily living skills, and personal care
- access to education and employment – support to continue learning or enter the workplace
- community and social inclusion – helping you stay connected, make friends, and take part in activities
How to access support
- if you already have a social worker, they will discuss your transition plan with you
- if you haven’t received support before but think you may need it, you can request an assessment
