How adult social care collaborates with young people to develop transition plans.
We believe that young people should be at the centre of their transition planning.
Our approach is collaborative, meaning we work with you, your family, and professionals to create a plan that reflects your needs, goals, and aspirations.
Your transition plan
Your transition plan is developed based on: your ambitions - what do you want to achieve in adulthood?
- your support needs - what type of care or assistance will help you reach your goals?
- your preferences - how do you want to be supported?
Who will be involved?
- you - your voice is the most important in planning your future.
- your family or carers - if you want them involved, they can help advocate for your needs.
- social workers - to assess and plan the right level of care.
- health professionals - if you have medical needs, they’ll be part of the discussion.
- education or employment advisors - to help with school, college, apprenticeships, or work opportunities.
What happens next?
- Meetings and discussions - you will have regular check-ins to plan your transition.
- Assessment (if needed) - if you qualify for adult social care, we will complete an assessment under the care act 2014.
- Creating your support plan - a tailored plan will be put in place to support your move into adulthood.
- Review and adjustments - your plan can be updated as your needs change.
