Use this form if we have asked you to submit evidence.

Before you complete the form

To upload evidence for business rates you will need to log into a My Lewisham account. If you don’t have one already, please click 'Log in and submit evidence' below, then choose 'register' and follow the instructions to create your account.

How to complete the form

After you log in you can complete the form:

• It should take between 10 and 20 minutes to complete.
• You will need to have your business rates account number and scanned copies of your evidence ready.
• You can upload scanned evidence or do this later.

What happens next

• We will send you an email confirmation.

• We will review your evidence within 10 working days if you have given us all the necessary evidence. We will contact you if we need more evidence.
