This form is for an adult who is caring for another adult. You can fill it in for yourself or on behalf of someone else.
Don't fill in the form if you're under 18 - see our information for young carers instead.
How long it will take
The assessment will take you about an hour to complete. Please allow enough time so that you don’t lose your answers.
What happens next
When you submit the form to us you'll get an automatic receipt. We'll be in touch within five working days.
