Apply for a Freedom Pass for disabled people

Who should use this form

You can use this form if you are eligible to apply for a disabled person's Freedom Pass.

Before you start

To apply for a Freedom Pass you need to log into your My Lewisham account. If you don’t have one already, please click 'Log in and apply for a Freedom Pass’ below, then choose 'register' and follow the instructions to create your account.

What you need

After you log in you can complete the form. It should take between five and 10 minutes to complete. You will need to upload:

• proof of identification

• proof of address

• proof of disability

• a recent passport-quality photo.

We will assess your application and post your Freedom Pass to you if you are eligible.
